When the Care System Goes Offline: Keeping the Paper Trail Straight
A practical approach to approved downtime records, secure handling, and careful reconciliation when a care documentation system becomes available again.

When the Screen Stops Working, the Shift Keeps Moving
A login failure or network outage can arrive in the middle of a busy shift. Staff still need to provide care, pass on concerns, and record what happened. The provider's downtime procedure should say which approved form to use, who to contact, and how records are secured until the system returns. Staff should not improvise with personal phones, unapproved apps, or loose notes that may be lost.
On the downtime record, make the original event time clear, identify the author, and write legibly using the approved process. If a time is approximate, say so. Keep each person's information on the correct form and follow the same privacy and access rules that apply to electronic records.
Don't Wait to Escalate an Urgent Concern
A system outage is not a reason to delay an emergency response or a required escalation. Use the provider's agreed phone or paper pathway, record who was contacted and when, and note the response. If the usual contact cannot be reached, follow the escalation chain in the downtime procedure.
Keep a simple log of when the outage began, which locations or systems were affected, and who took ownership of restoration or communication. That helps the next shift understand what is still outstanding without adding a second version of each care record.
When the System Comes Back
Follow the provider's reconciliation process rather than entering everything from memory. Transfer the information into the authorised record with the true event time and the actual entry time shown as required by the system and local procedure. Preserve the original paper record until the designated person confirms it has been reconciled and stored or disposed of correctly.
Check for duplicate entries, missing pages, unsigned forms, and tasks that were handed over but never closed. If something cannot be reconciled, raise it through the provider's record-integrity process; do not silently fill in a gap.
